Out of Network Insurance

VITA Physical Therapy and Wellness is an out of network insurance provider. This means that we have not entered into a contract with insurance providers to receive reimbursement at a specific contracted rate. In a cash-based model the patient pays at the time of service, you may pay for services using credit/debit card, HSA/FSA funds, check or cash. However, you are still able to submit your services to insurance for reimbursement. You will receive an invoice called a superbill at the time of service, this includes all the information (tax ID, NPI, business info, etc) , associated diagnosis (ICD-10) and billing (CPT) codes needed to submit your claim to your insurance company.

Many patients choose to receive services out of network in order to receive the specialized care they need. This benefits the patient because many insurance contracts place strict regulations and rates on the services delivered. In a cash-based model the patient and therapist enter into a contract to determine a treatment plan that will help their goals be reached in both a time and cost efficient manner, this avoids the insurance company dictating care. Ultimately it gives the patient the ability to make fully informed decisions about their health care decisions. These fees are well below the typical amount submitted to an insurance company for billed physical therapy services. Additionally, this can benefit those individuals with high deductibles. This allows VITA to focus all of our energy on providing the best patient care possible, while keeping administrative costs low. Here at VITA we are willing to negotiate rates if you are experiencing financial hardship, as we feel strongly that everyone should be able to receive the care they need. As with any other physical therapy business, documentation of evaluation, treatment visits, and progress notes are still completed after every visit and comply with all legal requirements.

Cancellation Policy: If you are unable keep your appointment, we kindly request 24 hours notice. If your appointment is canceled in less than 24 hours a $50 late charge will apply.
